Whether your child is a new or returning participant, completion of the registration process requires submission of (1) this completed registration form, (2) registration fee, (3) signed volunteer form (4) signed copy of code of conduct and equipment forms, (5) a certified copy of your child’s birth certificate, (6) a signed copy of the concussion awareness form, and (7) proof that your child has undergone a physical exam and has been cleared to participate. All necessary forms are available for you to read under the Registration tab on the home page. Flag Football/Cheer: 1st and 2nd grade. Registration fee is $150 due at this time. Plus $150 refundable volunteer fee will be due at the time of equipment pick up. Freshman Football: 3rd and 4th grade with a max weight limit of 140lbs. Registration fee is $300 due at this time. Plus $150 refundable volunteer fee will be due at the time of equipment pick up. Junior Varsity Football: 5th and 6th grade with a max weight limit of 170lbs. Registration fee is $300 due at this time. Plus $150 refundable volunteer fee will be due at the time of equipment pick up. Varsity Football/Cheer: 7th and 8th grade with a max weight limit of 210lbs. Registration fee is $300 due at this time. Plus $150 refundable volunteer fee will be due at the time of equipment pick up. *Ages and weight limit is per the SMYFA League.* *All forms you will be consenting to, are listed under the Registration tab on the website. **Registration Fees are NON-REFUNDABLE after June 1st.
